Job Title: Social Worker x6 Rate of Pay: ?24.00 per hour (Inside IR35) Hours: 37 hours per week Location: Hybrid Working/Sandwell Council House, Oldbury, B69 3DE. Start Date: ASAP Contract type: Temporary (Agency) 12 weeks with a view to extend Job Description and Personnel Specification are attached, which also provides more information above the role. Brief description: Are you a qualified Social Worker, registered with Social Work England who has experience of working in a busy hospital team/Intermediate Care? The team operate a 7-day service including Bank Holidays. We have vacancies within the Hospital 2 Home team which is an exciting opportunity for individuals who are flexible, innovative and have a proven track record of partnership working, with a can-do approach. If you can work under pressure and have a great attention to detail, then we would like to hear from you. We are looking for staff with a range of experience, with knowledge of social work practice and legislation including the Care Act 2014 and Mental Capacity Act 2005, a good understanding of personalisation and excellent assessment and support planning skills. You will work from various locations and will be responsible for supporting the timely discharge of people from hospital or intermediate care settings. You will be required to work in partnership with the NHS and the CCG and will have a strong customer focus. For further details please see Job Description and Person Specification.
