Notice Description

Invitations are now invited from suitably experienced and capable providers to deliver the provision of a Primary Mental Health Care Service. The service will assist the CCG to: Service requirements are summarised as follows: * A common assessment framework across the three new service elements (Primary Care Recovery, IAPT and Mental Wellbeing), and incorporating access to secondary mental health services * A seamless mental health care pathway, from primary care to secondary care * Services that are sensitive to 'transitions' particularly from child and adolescent mental health services (CAMHS) to adult mental health services, and from adult mental health services to, mental health services for older adults * Primary Care Recovery to support people with complex and or severe mental illness, but stable presentations, who need continued support to sustain recovery outside of secondary mental health services o Facilitating discharge from secondary care o Providing support to maintain recovery progress in primary care setting o Support clients and primary care professionals to promote clients' good physical health * Promote clients' social inclusion * IAPT to be integrated into physical health care pathways, with an initial focus on specific long term conditions o Cardiac o Respiratory o Diabetes * Mental Wellbeing Service working in partnership with non-mental health specialists in the borough, to * Support local residents with self-identified, and or formally diagnosed, mental health problems * Find opportunities, supports and services that promote social inclusion/reduce social isolation * Provide social and psycho-social support to help clients to maintain their mental wellbeing, and thereby prevent progress towards the need for treatment.
