Notice Description

BHR CCGs wish to commission 22 beds across 2 or more Nursing Homes within the Borough of Havering. The pilot D2A process will actively support the Home First model; returning home will always be considered as part of the discharge decision making process, only where this is not possible will D2A be considered. The D2A pathway is an interim placement rather than a longer-term placement and this will be communicated effectively to patients and carers/family members throughout the process. Referrals will be made onto the pathway via the trusted assessor approach to the CCG team who will then arrange the placement in one of the commissioned nursing homes. The prospective care home provider will need to: - Currently operate nursing home(s) within the Borough of Havering. - Have a CQC rating of good or outstanding from their most recent CQC inspection. - Able to confirm and assure the CCG that there are no ongoing safeguarding or CQC investigations regarding services within the home. - Able to confirm there are no suspensions or embargoes on your home from either CQC or the local authority. - Able to confirm your organisation is fully compliant with Data Security and Protection Toolkit (DSPT) requirements. - Confirm how many beds would your home(s) be willing to allocate solely for BHR CCG's Discharge to Assess Patients.
