To develop, deliver and evaluate a service to support healthy weight management and healthy living for adolescents aged 14 years to 19 Years ( up to 25 years SEND). The service will form an integral part of the childhood healthy weight management action plan providing a tier 2 weight management service for those needing addtional support. It will also contribute to a universal programme to enable and empower adolescents to improve their own health and wellbeing through enhancing knowledge, lifelong skills and building resilience. To acheive this goal, the service will incoprorate tailored support, mentoring and collaboration with local and community services.
