City & Hackney Enhanced Health Visiting Service
Notice Description
To provide a holistic Enhanced Health Visiting Service which provides expert advice, support and interventions to families with children in the first years of life, in line with the revised 2021 national Healthy Child Programme (HCP). The service must be 'universal in reach and personalised in response' and work with families with children aged 0 to 5yrs to identify health needs as early as possible, improve access to services and improve health and wellbeing by promoting health, preventing ill health and reducing inequalities in the populations of the London Borough of Hackney and the City of London. Service provision is to be provided at five levels: Level 1: Community Level 2: Universal - Mandated Reviews Antenatal New Baby 6-8 weeks 1 year a 18 month- desktop review of all child health records 2-2 1/2 year Level 3: Targeted One month 3-4 months 3 - 3 1/2 years Level 4: Specialist Multi-agency working Level 5: Intensive Home visiting service for vulnerable families with children under 2yrs, open to families regardless of age, number of children and not restricted to first pregnancies As the lead for the Healthy Child Programme 0- 5yrs, the Enhanced Health Visiting service must also have 9 High Impact roles in line with the needs of the City & Hackney population: Breastfeeding Transition to Parenthood Perinatal Mental Health Special additional needs Healthy Weight Substance Misuse A&E Liaison Nurse Lead for Integrated Reviews Homelessness
