Notice Description

Home to Decide is a service designed in conjunction with a provider, and initially piloted with Kent County Council and West Kent CCG. The Home to Decide service is designed for people aged 18 years and over and are determined by Adult Social Care and Health (ASCH) or the NHS as eligible and who are ordinarily resident within the administrative area of Kent County Council . It is imperative that people are at the heart of Health and ASCH activities receiving services that are easy to access, of good quality, that support their wellbeing and maximise their ability to live independently and safely in their community. Home First' is one of the key priorities NHS England communicated to Accident and Emergency delivery boards in 2016 and refreshed in the guidance underpinning the improved Better Care Fund High Impact Changes 2017. The focus of the Home to Decide service is to ensure the person is safe at home following discharge from hospital and aims to reduce residential care admissions through an intensive programme of support in the Person's own home.
