Notice Description

A clinically led audit of 1000 Barts Health patient records for patients that were readmitted within 30 days. For each patient in the sample, the review team should decide whether the readmission could have been avoided through actions that might have been taken by the provider, the primary care team, community health services or social services, or a body contracted to any of these organisations. Outputs of the audit will include: An initial meeting between the successful bidder and provider and commissioner representatives to agreed objectives and process and how the results with inform the outcome. Part 1: A case note review undertaken by the successful bidder's clinical team, on the provider premises, to identify those cases where the admission was potentially avoidable Part 2: A case note review undertaken with the successful bidder's clinical team, plus the trust and CCG clinicians, to discuss a sample of admissions deemed potentially avoidable/not avoidable during Part 1. A paper detailing the outcome of the clinical review to be shared with providers and commissioners identifying themes and areas for service development/improvement. A presentation of the findings by the successful bidder to the commissioners and provider. Additional information: The successful provider is expected to commence mobilisation of the project on 24 July 2017, with the actual project delivery commencing on 07 August 2017.
