DEAR PROVIDER - PLEASE NOTE. There has been a minor update made to the specification. This is reflected within the document 'ITT - UPDATED' which will be sent directly to you via correspondence. This 'ITT - UPDATED' document is to be used in place of the published 'ITT' document, and contains only minor changes to the specification section only. All other elements of the document remain the same. We will continue to send repeat communication to ensure the correct document ('ITT - UPDATED') is used for your return. Leeds & York Partnership NHS Foundation Trust (mHabitat Team) working on behalf of the Leeds West Primary Care Network, wish to procure a dynamic, informative, public facing web service for each member of the care network. Leeds West Primary Care Network are a group of 37 General Practices within the NHS Leeds West Clinical Commissioning Group. To make the process of engaging with local GP practices and their online services easier for service users, we want to develop a standard web service 'template' that could be used and customised by all of the 37 practices, while retaining the common template features and familiar approach.
