Family Focus Health Trainer

Notice Description

Family Focus is the offer from Cheshire East Council in respect of the national Troubled Families programme. This is to provide Outcome 5 of the Public Health target and support the Health and Wellbeing Strategy. The Family Focus Health Trainer provider(s) will: 1. Establish a Health Trainer Working Group 2. Implementation of the Health Trainer pathway 3. Identification and implementation of the referral criteria and protocol, including exclusions (A family will need to consent to being referred into the programme otherwise this will be considered an exclusion. If the Health Trainer Provider(s) feels that the referred individual does not have the motivation to change due their lifestyle or a particular inhibitor in their circumstances or that they present a risk to themselves or others this can be considered an exclusion and would need to be referred back to the key worker with the reasoning) 4. Design and implement referral documents, health trainer documents, and promotional materials 5. Develop a monitoring and evaluation processes to monitor progress of individuals Additional information: Submissions must be received via The Chest https://www.The-Chest.org.uk The project reference on The Chest is DN125171. Please note you are required to register, express an interest, download the Request For Quotation (RFQ) documentation and upload the completed RFQ documentation by the below deadline. PLEASE NOTE RESPONSES MUST BE COMPLETED AND RETURNED VIA THE CHEST BY 12 NOON ON THE 27th of MAY 2016. Please ensure you leave sufficient time to upload your submission prior to the closing date/time. The council cannot be held responsible for technical/ICT issues in leaving the uploading of your submission too late. No submission received after this closing date and time will be considered other than where there are exceptional circumstances which may be considered by the Borough Solicitor in his sole discretion. Please note that submissions which are partly through being uploaded at the closing time will be considered to have not been received. The performance of the contract will be within the boundaries of Cheshire East.
