Notice Description

The overall approach for Housing First is to provide a stable, independent home and intensive personalised support and case management to people with multiple complex needs who have experienced homelessness and are at risk of rough sleeping and who have faced multiple exclusions. The accommodation will be offered to individuals who have been identified as homeless or at risk of homelessness with a serious mental illness including those with histories of entrenched or repeat homelessness. The overall purpose of the Service is to provide an alternative supported accommodation option for people where alternative options for accommodation have not worked. For example, they have not been able to thrive in an environment with multiple occupancy and for those individuals who have a history of circling housing and health systems and returning back on to the streets. The service will provide a tailored and personalised support package for each individual in the accommodation to enable successful maintenance of their tenancies.
