Carers - Integrated Service

Notice Description

The Council and Clinical Commissioning Group partners are looking to commission a fully integrated model of support for Carers. This will deliver a seamless service where there is a single phone number for all commissioned Carers services in Cheshire West and Chester borough. We wish to commission a lead organisation that will i) deliver a generic information and advice service ii) manage the delivery of a variety of services delivered by a range of organisations; and iii) manage the carer break services commissioned by the Council and Clinical Commissioning Groups. There will be a single point of access where Carers have a triaged assessment and are offered a range of appropriate services dependant on their needs.
