Provision of a hospital discharge service in North Staffordshire (Excluding Stoke on Trent). The hospitals are University Hospitals North Midlands NHS Trust and County Hospital, Stafford. There will be an estimated 441 referrals made to the service. The Service is to run from 1st April 2022 - 31st March 2023
