Provision of Crisis Service in Central Lancashire
Notice Description
The Crisis Service forms part of the intermediate care service which is a range of integrated services, and supports the promotion of independence and faster recovery from illness. The Service aims to prevent unnecessary acute hospital admission, premature admission to residential care, support timely discharge from hospital, to provide urgent care and support in crisis situations such as carer breakdown. The Service is to offer care and support for adults in their own homes when an acute situation or crisis occurs and who without additional support would normally be admitted to hospital or residential care. The Service can also be accessed to assist informal carers who may be experiencing a period of disability that prevents them from continuing with their normal caring role on a short-term basis or who are having difficulty coping in their caring role. The Service will be available 24 hours per day, 365 days per year and has to be made available to many Service Users at the same time in different geographical locations across Central Lancashire.
