The service support clients to lead independent and fulfilling lives, promote effective personal support networks enabling them to successfully maintain their own home, within the community. The provision of person-centred support reduces and/ delays the need for care provision, residential or nursing accommodation, and lessens the need for crisis intervention by health and social care.
