Notice Description
The service will support clients to lead independent and fulfilling lives, promote effective personal support networks enabling them to successfully maintain their own home, within the community and their chosen way of life. The provision of person-centred support will reduce or delay the need for care provision, including residential or nursing accommodation, and lessen the need for crisis intervention, including health and social care provisions.
