Notice Description

General Overview and Objectives of Services The key aims of the service are: Deliver a holistic primary care and social care service to the following groups: * Homeless; * asylum seekers; * refugees; and * socially excluded groups Deliver integrated, outreach services to hard to reach groups. These services will be interlinked. The outreach services are: * respite and integrated care and support service * pathway service (holistic assessment of health and social care needs of inpatients) * street medicine service The key objectives are: * to provide safe, high quality, effective structured primary medical care and health promotion, tailored to the unique needs of Bradford's most socially excluded patients, that is responsive to Patient and community needs; * to focus service provision on 'predict and prevent' as well as 'diagnose and treat' - placing a strong emphasis on improving capacity to anticipate the future health needs of this population whilst also taking into account current needs; * to increase access to primary medical care, health promotion and Patient education that proactively maximises engagement with hard to reach and socially excluded individuals; and to develop approaches that empower Patients, their carers and families; * to work in partnership with other agencies to address wider Patient needs; and * to strive continuously through innovation and engagement to improve Patient experience and outcomes for primary medical care Patients, their carers and families.
