Notice Description

This is an ITT for the provision of Intermediate Care Beds for South Norfolk Clinical Commissioning Group (SNCCG). This procurement will provide intermediate care for people who are considered unsafe to remain in or return to their own homes but who would have the capacity to live independently within their own home if provided with suitable interim support; i.e., it will help prevent mainly frail older people transferring to long-term care directly from an acute hospital. The Beds shall provide step-down care from acute hospitals and step-up care from the patients' usual place of residence; i.e., to provide early supported discharge and an alternative to hospital admission. Further assessment and rehabilitation of older people will be a key element of the provision. This service is in addition to, and different from, the existing NHS-run Intermediate Care services ("community hospitals"). People admitted to the Service should move through the system as efficiently and safely as possible, with the expected average duration of placements being for approximately 2 to 3 weeks. This ITT is for providers in a ranking order to provide a combination of a total of 6 Block beds per week, and a multi-provider agreement for an undefined (flexible) number of spot-purchased Intermediate Care Beds. The term is for 12 months and the estimated Block value is PS220,000 to PS234,000 per year. Additional information: The key expected outcomes of the service are to: * Ensure people have a comprehensive assessment and structured individual care plan that involves active rehabilitation (e.g., physiotherapy, occupational therapy) where appropriate or opportunity for recovery that is sustainable once the patient has returned to their usual place of residence; * Deliver short-term intervention to enable users to optimise their independence evidenced by recording of patients' Activities of Daily Living (using the Modified Barthel Index both on admission and discharge, and the Rehab Complexity Score on admission); * Reduce the length of patient stay (LOS) in the acute hospitals; * Reduce LOS in the Intermediate Care Beds; * Avoid unnecessary hospital admissions; * Support people with end of life care where there is an identified specific symptom, medication control goal or rehabilitation need that can be met within a limited period of time; * Ensure that the majority of patients are discharged within an average of 2 to 3 weeks; * Ensure smooth transition into the Service and, at discharge, out of the service to other care providers to enable a seamless experience for the patient.
