Parkinson's Disease Nurse Service in Brent

Notice Description

NHS Brent Clinical Commissioning Group (CCG) is seeking quotations from providers who are interested in providing a Parkinson's Disease (PD) Nurse Service for people registered with GPs in the borough of Brent. The new community service will entail the employment of a Band 7 or equivalent nurse who will provide care and treatment for PD patients in the community in close collaboration with secondary care neurology consultants and primary care Networks. It is expected the post holder will start employment by 1st November 2016. Parkinson's UK and Brent CCG will need to be involved in the recruitment, with it being a condition that a Parkinson's UK representative is included on the recruitment panel. Although the PD Nurse will be expected to have an office base in the borough of Brent they will be expected to spend the majority of their time working out in the community visiting patients wherever is appropriate, including homes (Residential and domiciliary) as well GP practices and community clinics. The successful provider will be expected to provide the nurse with IT and telephony facilities and administrative support where appropriate. The PD nurse will act as a catalyst for developing services, pulling together a multidisciplinary team to manage people with PD. Early initiatives would include a review of all people with PD currently accessing services to identify needs and gaps in provision, creating a register of patients with PD, setting up of newly-diagnosed groups and PD -specific expert patient groups. The PD nurse would be the first point of contact for patients, carers and professionals in addition to the other roles outlined above. The contract will be for a two year period commencing from when the Parkinson's Nurse is in employment. The maximum value across the two year period is PS122,500.00. Following this period, and a robust evaluation process, the CCG reserves the right to extend the contract for a further two years at same value.
