Community COPD Service - Invitation to Bidder Information Event

Notice Description

NHS Halton CCG has a requirement for a Community COPD Service. Operating across Halton, the Community COPD Service will provide home (including residential & nursing homes) and community based care. The service must be able to deliver: * Patient centred care which allows patients with COPD to be more independent, taking more responsibility for their own care and quality of life. * Seamless care and support to patients and their carers through close working arrangements with social care and 3rd sector providers * A 'shift of care' into the community, working primarily with general practice as part of a multi-disciplinary team designed around, and for, the patient and carer * Referrals of appropriate patients to the Community Pulmonary Rehabilitation Service The Community COPD Service will also be required to make a significant contribution to NHS Halton CCG's plans to reduce non-elective admissions, to effectively manage prescribing costs and to improve the management of patients with COPD in primary care. The contract will be awarded for an initial 5-year period with the potential to extend for a further 1 x 2-year period. ** Prior registration for the event is essential ** The bidder information event will take place on Thursday 18th May 2017 at 1.30pm at The Heath Business & Technical Park (room Sycamore A), Runcorn, Cheshire WA7 4QX. The event will consist of presentations by the CCG about the service which will be followed by a 1 to 1 meeting opportunity with members of the CCG. These meetings will be allocated on a first come, first come served basis and the CCG will make all endeavours that all interested parties are provided with a 1 to 1 meeting.
