West Essex Transfer of Care (Patient Flow) IT System - Engagement
Notice Description
West Essex Clinical Commissioning Group (WECCG), on behalf of the West Essex system, is inviting suitably qualified providers to attend a market engagement event which will inform the West Essex systems intention to commission a Patient Tracking Software system. The West Essex Health & Care system (ICP) has an ambitious plan to extend the scope of our Single Point of Access into a Care Co-ordination Centre operating 24/7. Care Co-ordination will be both pro-active case management, based on risk stratification, and real time reactive to referrals received. The Centre will hold a single integrated view of all system capacity to enable referrals to be prioritised and allocated on needs basis. The need for Patient Tracking Software has been identified as a key component for the delivery of the Care Co-ordination Centre. In order to secure a viable IT solution, at a cost sustainable to the System, we are keen to secure support from a business partner to develop a business case/proof of concept to evidence cost benefit on IT investment vs development of existing manual tracking solutions. The IT solution is required to run in parallel with existing systems, accepting direct feeds from each clinical system based on user input, and defining a state or relationship change with the patient throughout their journey. As such, this change of state would define an output within the CCC dashboard and trigger actions required to allocate resources to meet patient needs. Existing systems in use include SHREWD, Cosmic, SystmOne, EMIS and Mosaic. The IT solution is required to support real-time allocation of referral across all intermediate care services but as a minimum must include: - Discharge from acute hospital beds - Admission and discharge from community health and community mental health beds - Admission and discharge from intermediate/interim beds in care homes - Admission and discharge from reablement at home services The West Essex Local Delivery Board has approved the development of a business case to determine the level of financial investment required and level of associated benefits, both financial and non-financial. The CCG are looking for a single provider to work in partnership with the West Essex system to deliver the business case which will be discussed at the West Essex Local Delivery Board and used to inform the systems future procurement options for the patient tracking system itself. The format of the event will be 1:1 discussions with interested providers which will take place on Tuesday 27th August between 12noon and 5pm and Wednesday 28th August between 2.00pm and 5pm.
