Primary Care Patient Coordinator Service

Notice Description

NHS Hounslow Clinical Commissioning Group(HCCG) intends to commission a Primary Care Patient Coordinator Service. The CCG would like to invite providers with an appropriate level of capability and capacity to express their interest in an opportunity to deliver this service. The aim of the Primary Care Patient Coordinator Service is to support the delivery of care for patients registered with Hounslow CCG GP Practices by providing coordinating staff to enhance and improve pro-active care, prevention and self-care. The core requirements of the service are to: o Identify patients requiring care planning through approved risk stratification processes. o Work with local health and social care teams to plan and organise regular Multidisciplinary Team (MDT) Meetings. o Review and follow up MDT actions. o Provide administration support for MDT meetings at practice and locality level o Support the effective utilisation of the Patient Activation Measure o Support the production and implementation of standard operating procedures and policies for processes such as call and recall and MDT meetings. o Support the effective utilisation of the Whole Systems Integrated Care dashboard o Participate in quarterly action learning sets to discuss issues and share good practice. o Monitor and audit attendance rates HCCG anticipates awarding a 2 year contract with the option to extend for a further year at their discretion. The anticipated annual contract value is circa PS660,000. Should HCCG decide to undertake a formal procurement process, this is anticipated to commence in October 2016 working towards full service commencement early in 2017. The responses to this Market Engagement invitation will inform HCCG's commissioning intentions. Responses will help to assess whether or not to competitively tender this contract and, if so, how such a process may need to be designed. ***Providers are invited to express their interest in this potential opportunity by returning the attached questionnaire to lorraine.shaw4@nhs.net no later than 12 noon on Friday 5th August 2016***
