Notice Description

The Short Term Support in the Community service will ensure that people who would not benefit from our current Reablement at Home service will be able to access support quickly, and that their longer term needs are assessed when their situation has stabilised rather than at the point of crisis. As such the new service will form part of a simplified pathway, complementing and working alongside the Reablement at Home service and Longer Term support services. People who would benefit from this service include those adults with complex health and social care needs such as advanced dementia or challenging behaviour, and may not have the same potential to regain their independence. The new service will be for up to a maximum of six weeks and will: * provide health and social care support to anyone aged over 18 who would not benefit from our Reablement at Home service; * provide support to someone who is at crisis point living in the community, including where normal care arrangements (including unpaid care provided by family or friends) have unexpectedly broken down and an immediate response is required; * ensure that assessment of on-going care and support needs is made at the right time and in the right place. This should be at the customer's home in an environment which is familiar for them, and not when they are in crisis. The service will also support our new Good Lives Approach to social care practice, building upon a person's assets and their existing support networks.
