NHS Wandsworth & Merton CCG - Managed Adult Continuing Healthcare Service
Notice Description
Invitations are now invited from suitably experienced and capable providers to deliver the provision of a Managed Adult Continuing Healthcare (CHC) Service. Two separate services are being integrated into a singular contractual solution to support NHS Wandsworth and NHS Merton Clinical Commissioning Groups (CCGs) who will act as Contracting Authority. The CCGs are offering a contract for 3 years with an option for this to be extended for up to a further 2 years, with the new integrated service intended to commence from 1st April 2019. The service will support the CCGs to discharge their lawful obligations to ensure the meeting of assessed needs of adult individuals having entitlement to receive care which is fully funded under the National Framework for NHS Continuing Healthcare and NHS Funded Care. The service will assist the CCGs to: * Prevent people from dying prematurely * Enhance quality of life for people with long term conditions * Help people to recover from episodes of ill health or following injury * Ensure people have a positive experience of care * Treat and care for people in safe environments and protecting them from avoidable harm. The service will provide a coordination of managed service for packages of care provided both within care facilities and within patient's own homes. Full details about this service are provided within a suite of procurement documents which in addition to competitive procurement tender documents, includes a Service Specification and a supplementary Memorandum of Information to ensure potential bidders are fully informed as to the scope and intentions for this service and the commercial arrangements intended to support the contract.
