Notice Description

In order to inform thinking related to digital, Leeds Clinical Commissioning Group are keen to understand what the market has to offer in terms of digital solutions for people living with diabetes, those caring for someone with diabetes or for those working in diabetes-related services such as specialist diabetes teams and primary care. Leeds Clinical Commissioning Group are interested to hear from providers who can offer one or more of the four layers of provision outlined below that will meet the needs of all demographics across the city: Information - via website offering Education and Advice, documented pathways E-learning - for healthcare professionals as well as people living with diabetes or caring for someone with diabetes Tracking of health status - including tracking of bloods, medications, etc. Clinical Management - tool for healthcare professionals to respond to tracking of health status These solutions will need to support the achievement of the following outcomes, with an emphasis on self-management, integrated and proactive care: - Improved motivation and confidence to self- manage - Improved skills and empowerment of all healthcare professionals delivering Diabetes care - Prevention and/or reduction of diabetic complications - Improved treatment targets (blood pressure, HbA1C and cholesterol) - Improved patient choice We ask that interested providers submit responses to the key questions using the questionnaire template provided in the Current Opportunities and Notices page at https://www.nhssourcing.co.uk Respondents are requested to specifically address the questions rather than attaching extensive technical or promotional information as such information will not be considered. Supplementary information pertaining to the achievement of the aforementioned outcomes will be considered but restricted to a maximum of 250 words. Information provided will form the basis of selecting providers to attend an event on 30th January in Leeds to present their solutions to healthcare professionals and commissioners from across Yorkshire & the Humber, which is being hosted by the Yorkshire & Humber Academic Health Science Network (YH AHSN). Please complete the template and return via the NHS Sourcing portal by 12.00pm, on Friday 14 December NB: Please be aware that involvement in this process is purely a market engagement exercise to gather facts.
