Notice Description

Homelessness is a key issue in Central London CCG and a range of services exist to help tackle and manage this multifaceted problem, and to help further the health and wellbeing of homeless patients in the borough. The CCG is looking to procure a peer advocacy and care navigation service for homeless patients. The main aims of the service are to offer one-to-one engagement to all street homeless people and homeless hostel-dwellers across Central London CCG geography, enabling them to make, attend and gain value from health appointments. The service will also encourage them to engage with, and make appropriate use of health services, and to lead more independent lives. The service will also work with homeless patients specifically identified as requiring special care and attention, helping to facilitate and coordinate their care, and to navigate them around the system. This will involve collaborative working with other organisations including, but not restricted to, homeless hostels and the two designated homeless GP practices in Central London. A contract for the provision of this service will be awarded to the successful provider for 12 months. The expected service commencement date is anticipated to be 1 November 2017.
