Notice Description

This contract is for the provision of specialist services for young people and adults with a Learning Disability including Autism and/or Mental Health care and support needs, whether funded under social care legislation, as NHS Continuing Health Care, or as aftercare under Section 117 of the Mental Health Act. The aim of the Service is to support Service Users to live independently in the home of their choice for as long as possible and to enable them to lead a good quality of life. This Flexible Purchasing Solution will comprise the following three Lots, according to the primary need of the young person or adult. This procurement will follow the Open Procedure under the Light Touch Regime of the Public Contact Regulations 2015.

Lot Information

Lot 1 - Young people aged 16 to 18 years and adults with mild or moderate level support needs

Service Users may: have a mild or moderate learning disability, autism, mental health condition or forensic behaviors and require a level of support; have an additional physical disability or health care need; need support up to 24 hours over 7 days per week but this could be shared support with 1:1 support built into their care package where necessary

Renewal: At any point that the commissioning authority deems it to be necessary.

Lot 2 - Young people aged 16 to 18 years and adults with complex support needs.

Service Users may have complex needs that could include a learning disability, autism, mental health condition including personality disorder and schizophrenia or exhibit forensic behaviours and require a higher level of support. Service Users may also have an additional physical disability or health need.

Renewal: At any point that the commissioning authority deems it to be necessary.

Lot 3 - Young people aged 16 to 18 years and adults with highly complex autism or very challenging support needs

Service Users may: have a highly complex or very specific support need that could include a learning disability, complex autism, mental health condition or forensic behaviours and require a higher level of support. Service Users may also have an additional physical disability or health need; have a personality or psychological disorder; have a severe or complex learning disability, and/or mental health condition and/or autism and/or long term health conditions where medical intervention may be required

Renewal: At any point that the commissioning authority deems it to be necessary.
