This contract is for the Provision of Intermediate Care Plus (IC+) Beds within County Durham via a block contract arrangement. Lot 1A EASTLot 1 A East for IC+ Block Beds;- The service will: * Promote rehabilitation, recovery and recuperation enabling service users to attain the optimum level of independence through the provision of both personal care and practical support. * Support service users who would otherwise face unnecessary, prolonged hospital stays or avoidable admissions to acute care, long-term residential care or high-level independent care packages. * Provide a setting for an intermediate level of care over a short time spell which minimises the risk of a person being re-admitted to a hospital due to an untimely or poorly planned discharge to their own home. * Facilitate and allow for continuing supervision in a more appropriate setting when an acute hospital bed is no longer required. * Provide a service that is time-limited, normally lasting no longer than 6 weeks and often much less. * Provide, on the basis of a documented, structured, individual care plan, input that maximises independence and enables service users to live at home as independently as possible. * Expedite planned, effective early hospital discharge into the service.
