The aim of this service is to improve the long term clinical, lifestyle and psychosocial outcomes for people diagnosed with type 2 diabetes through the provision of on-going, individualised self-management education to facilitate the knowledge, skills and behaviour change necessary for diabetes self-care. The objectives are to: * Undertake a learning needs assessment to understand the requirements, goals, and life experiences of the person with diabetes. * Undertake an initial assessment of individuals knowledge, skills, and confidence to manage their own health using a validated tool e.g., Patient Activation Measure (PAM). * Develop a personalised plan of education to tailor support to 'meet individuals where they are' and to enable to service to determine what to deliver to influence people's levels of activation. * Support informed decision making to enable individuals to be aware of their choices relating to the management of their diabetes, the potential outcomes of these choices and to have their personal values considered in decisions about their care. * Improve self-care behaviours relating to knowledge, attitudes, communication, family support, financing, self-efficacy, and motivation to improve quality of life. * Develop individual's problem-solving skills and capabilities to enhance independence and resilience. * Encourage active collaboration with individual's health care team. * Offer ongoing access to education offerings via a variety of approaches to enhance engagement in long term diabetes self-care. Additional information: The Contract will be for an initial term of 3 years, with an option to extend for any period up to a further 2 years. The services are education and healthcare services falling within Schedule 3 to the Public Contracts Regulations 2015 ("the Regulations") which are not subject to the full regime of the Regulations, but is instead governed by the "Light Touch Regime" contained within Chapter 3, Section 7 of the Regulations (Regulations 74 to 77).
