The service will: - Facilitate timely hospital discharges, providing suitable temporary and short-term care packages. - Meet the needs of people requiring home support to live independently including short term reablement support. - Help avoid admission into hospital. - Help people move to long-term service provider quickly and with minimal disruption to their support plan and daily routines. The Contract will start on 01/11/2022 and end on 31/10//2023 with options to extend for 2 further periods of 12 months.",
