Notice Description

The Council has a priority for a Thriving Sustainable Cornwall that offers a secure home for all - this work directly contributes to this priority by bringing people in from the streets and supporting them into more sustainable move-on accommodation. The Mental Health service supports both of these objectives. The Council has a further priority of Vibrant, Safe, Supported Communities where health starts at home. This work directly contributes to this priority in a number of ways: rough sleeping both places individuals at risk of crime but also makes communities feel less safe due to its visibility and associated or perceived behaviours (street drinking, anti-social behaviour etc). By addressing and reducing rough sleeping and by meeting the wider needs of rough sleepers, communities feel safer and individuals are supported in safer environments. In addition, there are other projects that focus on improving the health of people who sleep rough, through addressing substance misuse, supporting hospital discharge and supporting complex females around sexual abuse and sexual violence. This project links with a wide range of accommodation and support services for rough sleepers. These services links to the Council's priorities in the Homelessness and Rough Sleeping Strategy which include prevention, intervention and recovery from rough sleeping with this service forming part of a wider pathway for rough sleepers from the streets into their own tenancies. The Next Steps team who will manage and support this provision work very closely with current CHL services with all referrals going through the Rough Sleeping Team in CHL. This will adjust to the new County Wide Hub in the RSI 5 service delivery model. This Next Steps service also works in partnership with the mainstream Mental Health and Adult Social Care services, Drug and Alcohol, Safeguarding, and Domestic Abuse Services. Employment and Training and general social inclusion will also be considered, and links will be made with local Voluntary Sector organisations. This service will also be closely aligned to Community Safety within the council and the wider Criminal Justice service ensuring a critical contribution and effective multi-agency working across a vast partnership of services. The operational team link with the Rough Sleeper Operational Groups and Safer Towns as appropriate.

Next Steps Rough Sleeper Mental Health Support

In delivering Lot 1, the Service Provider shall: * Offer mental health and wellbeing support across a range of domains such as housing, employment, finance and wider lifestyle areas, making referrals to other specialist agencies as appropriate; * Deliver an assertive outreach approach to rough sleepers as well as in-reach support for those who continue to require support once accommodation has been secured; * Form a key element of the multi-disciplinary team (MDT) to provide support to rough sleepers; those newly accessing temporary/supported accommodation and those who are about to leave supported accommodation; * Assist Service Users to improve their social and digital inclusion, helping them to: o Develop their own social networks, renew links with family, friends, peer support and volunteer groups; and o Increase their digital inclusion; participation in and contribution to the digital world. * Support individuals around their health and wellbeing, enabling them to move from the streets into accommodation, resulting in: o Appropriate access to formal Mental Health services; o Reduced calls to 'Blue Light' services; o Reduced admissions to hospital and other similar institutions; o Appropriate access to drug and alcohol services; and o Better prospects for moving on into independent living accommodation. * Support team members who are part of the MDT; and * Support Service Users through increased welfare advice, helping the development of healthy lifestyle skills and accessing money management advice. * Maintain up to date knowledge of legislation, national and local policies and issues in relation to Homelessness.

Renewal: There is to be one extension allowed for a further two year period subject to funding. Total contract length: 2 years plus 2 year extension = 4 year (48 months)

Next Steps Rough Sleeper Prescribing

In delivering Lot 2, the Service Provider shall provide all aspects of Lot 1 above, in addition to, but not limited to the following: Clinical Practitioner Nurse: * Offering Clinical support, assessment and medication as required; * Completing specialist mental health assessments and developing pharmacological treatment plans as required; * Monitoring interventions; * Working closely with the other service providers, ensuring links to wider mental health services, GPS, primary care practitioners, community matrons, pharmacists and statutory and voluntary agencies as required; * Provide comprehensive specialist assessments using highly developed skills, including the risk assessment and management of individuals who are cared for; * Providing evidence-based individual therapy, such as cognitive behaviour therapy for depression and anxiety; and * Registered with the Nursing & Midwifery Council (NMC) Clinical Psychological: * Implementing a range of psychological interventions for those accessing the service; * Provide psychological assessments of clients referred; * Provide expertise and specialist psychological advice, guidance and consultation to other professionals contributing directly to the service user/support system/provider organisations/criminal justice agency's formulation, diagnosis and treatment plan; * Formulate and implement plans for the formal psychological treatment and/or management of a person's problems; * Monitor and evaluate the progress of clients under their care and during the course of multi-disciplinary care; * Provide expertise, advice and support to facilitate the effect and appropriate provision of psychological care by all members of the treatment team; and * Registered with the British Psychological Society (BPS).

Renewal: There is to be two extensions allowed for a two further two-year periods subject to funding with annual break clauses should funding only be awarded for one year. Total contract length: 2 years plus 2 x 2-year extension options = 6 year (72 months)
