This provider will be responsiblea range of functions which are summarised below: 2 / 4 * Marketing, outreach, relationship management with professionals and use of risk stratification * Screening and risk assessment of individuals * Delivery of programmes in relation to Weight Management, Physical Activity and Falls Prevention, Smoking(specialist and community), Mental Wellbeing (brief intervention) as well as Innovation Projects. * Case management of participants and tracking of outcomes via an appropriate IT system.
