The Council wishes to commission the provision of 7 block contracts for building based specialist day centres and support for older people with complex needs within the geographical areas of: - Stafford Borough District, - Cannock Chase District -- Block A; -- Block B. - South Staffordshire District, - Tamworth District, - Lichfield District, - East Staffordshire.

The provider(s) will be expected to provide: - tailored services and support arrangements to meet people's individual needs and aspirations, - flexible operating times, supporting the need for respite, supporting carers to have a break at a time to suit them, - a range of stimulating activities for older people to participate in, including outreach facilities with well connected links with community-based resources, - appropriately skilled staff to care for people with critical/complex needs including challenging behaviour, ensuring for example that all individuals with dementia are supported through the pathway, - assistance to access an infrastructure of professional support and services, - transport to and from the day centre where there is an assessed need, - signposting to other services where appropriate. The service will: - improve and maintain individuals' health and well being through increased independence, choice, control, dignity and quality of life, - promote independence and enable the person to be an active member of society and the community, - provide a trusted environment for individuals providing reassurance to family and carers, enabling them to make informed choices about the care they receive, - provide a person centred and holistic service, responsive to the needs and circumstances of individuals, - maximise independence after illness or disability through working within a rehabilitation, re-ablement and enablement philosophy, - provide a high-quality service in terms of access, the individuals experience, and outcomes based on evidence of effectiveness, - provide a modern and flexible service, planned and designed, putting individuals at the centre, - increased end of life support for service users, increased support for carers and confidence and peace of mind for families and carers, - increased levels of service user satisfaction, enabling service users and their carers to have a real say in how support and services will be developed in the future, ensuring that people are at the heart of any decision that affects their life, - promotion of best practice, both locally and nationally, - provision of clear information on the cost and quality of services, so that people can make informed choices.
