Provision of general practitioner medical care to the inpatients of the Windsor Intermediate Care Unit <br/>Intermediate care services in Milton Keynes are offered through a range of integrated services that promote faster recovery from illness, prevent avoidable admission to an acute hospital or long-term residential care, support timely discharge from hospital and maximise independent living.<br/><br/>The Service is provided on the basis of a comprehensive assessment, resulting in a structured individual care plan that involves active therapy, treatment or opportunity for recovery.<br/>The overall aim of the service is to provide a reablement focus.

* Avoidance of admission to residential care.
* The realisation of the individual's full potential.
* The individual's return to living in the community.
