Innovative Platform for Home Based Services - Lot 5 Extra Care Services
Notice Description
Following the successful implementation of the Innovative Platform for Home Based Services. Southampton City Council invited competitive tenders from existing Lot 5 Platform Providers who were eligible to submit a bid (providers who applied to the join the platform and whose tender application was subsequently evaluated and accepted, prior to January 2024) for the delivery of care and support within an Extra Care housing setting, as per the following Sub Lots: Contract A - Graylings and Erskine, Contract B - Potters Court and Rozel and Contract C - Manston and Rosebrook with the overall aim of enabling people with a range of care and support needs to access care at any time, thereby providing a home for life.
Lot Information
Lot 1 - Homecare

Lot 2 - Live in Care Services24/7 care within an individual's home to support people in accordance with their care plan. Live-In Care includes the following sub-lots. Lot 2 a. Short term Live-In Care are Live-In packages that are required for two or less weeks. Lot 2 b. Medium term Live-In Care are Live-In packages that are required for up to four weeks. Lot 2 c. Long term Live-In Care are Live-In packages that are required for five or more weeks.
Options: The platform will be open for an initial period of 10 years with the option for extension up to a further period of up to 10 years, subject to periodic reviews.
Lot 3 - Continuing Health Care (CHC) and Complex Care for Adults and Continuing Care (CC) and Complex Care for Children Lot No 3be eligible, individuals must: * Live within Southampton City Council boundaries and be registered with a Southampton City GP practice. This is mandatory and both elements must apply for an individual to be eligible for this Service. * Be found to be eligible for Continuing Healthcare (CHC) or Continuing Care (CC) by NHS Southampton City place team within Hampshire and Isle of Wight ICB, or * Be found to be eligible for without prejudice or other temporary funding until assessment of CHC or CC eligibility is completed. Individuals who are eligible for CHC or CC may have a range of needs and must undertake an assessment as described in the tender documentation. Additional information: The estimated aggregate value for Lot 3 over 20 years (10+10) is PS332,656,741 this incorporates a 1.5% Rate rise per year, accumulative and 1.5% Population increase per year, accumulative

Lot 4 - Bridging ServicesThere is a need to support timely discharge from hospital in an enabling way which supports and maintains the wider health system, and to ensure that people experiencing a health crisis in their own home can be supported to stay at home and recover whenever possible. This Service aims to do this by facilitating a city-wide rapid discharge from hospital, maintaining people within their own home and avoiding hospital admission. Additional information: The estimated aggregate value for Lot 4 over 20 years (10+10) is PS30,884,903this incorporates a 1.5% Rate rise per year, accumulative and 1.5% Population increase per year, accumulative.

Lot 5 - Extra CareExtra Care housing schemes are a type of 'housing with care' where residents live in their own homes, with their own tenancy. Some residents are likely to have individual packages of care set up to meet their needs. There will typically be a staff presence on site 24 hours per day. Extra Care is mainly targeted at those over 55 but may also be used to meet the needs of younger adults with physical disabilities, learning disabilities or mental health conditions. Additional information: The estimated aggregate value for Lot 5 over 20 years (10+10) is PS64,736,932 this incorporates a 1.5% Rate rise per year, accumulative and 1.5% Population increase per year, accumulative.

"text": "The contract opportunity is divided into five lots which reflect the Services to be delivered under the Platform. It is anticipated that the platform will operate for at least ten years; however, the platform could run beyond this term for a further period of ten years (5th June 2023 - 4th June 2043). The operation of the Platform will be reviewed by the Authority, Platform at a minimum of every three (3), five (5), eight (8) and ten (10) years of its ten (10) year cycle(s). There is no limit to the number of providers who can be awarded onto to the Platform insofar as the requirements of the Authority are met. The Authority reserves the right to modify the platform during its operation. The Authority also reserves the right to terminate the operation of the platform at anytime. Providers can express an interest to join the platform or complete and submit the online questionnaire associated to this tendering opportunity at any time during the lifetime of the platform. Please note that the platform (the Council's arrangement for the procurement of Services for the Innovative Platform for Home Based Services) commenced with effect from 5th June 2023, and is currently operating. This notice relates to re-opening the tender opportunity for new-perspective applicants. \"Provider Commencement Date\" This is the date that a Provider is officially awarded onto the Platform. \"Platform Commencement Date\" This is the date the Platform commences and is open for awarding Orders The Council is subsequently communicating/clarifying intended timescales for evaluation of tenders for the remaining term of the framework, please note that this is applicable for existing providers who have already been appointed to the framework for either a change to a current offer or submission of a new tender application (for example for additional branch(es) to be added to the Platform or for existing branch (es) to deliver additional lots) and new/perspective providers. Please refer to the tender for further information and documents and we recommend you review information relating to - how the platform will work, contract management and award documents. We would like to advise that when a new tender is submitted it will be evaluated via an evaluation panel on behalf of the Council, during the following months of February, May, August and November and that this pattern will be repeated throughout the term of the framework. Please note for tenders to be evaluated during these months, tenders must be received prior to the last working day of the month, all further tenders received after this date will be evaluated during the next period of evaluation."
