You are invited to attend the market engagement event and should visit https://health-family.force.com/s/Welcome where you will be able to find further information regarding the service along with a short questionnaire for you to complete by searching for reference C234861 " Community Offer and Intermediate Care Support Service - Market Engagement".
An invitation will be issued upon response with your allocated time slot on that day. To confirm the time slots will be between 12:00noon and 04:00pm, dependant on the number of responses received.
