An integrated health care service to include: * GP and primary care service inclusive of dentistry, optometry, podiatry, physiotherapy, sexual health, public health services and other therapeutic services. * Fully inclusive mental health services both primary and secondary care services. * Fully integrated substance misuse service NHS England commissioners are intending to award a contract to an existing provider following a Direct Award process C. The lifetime value of the contract is PS9,146,978. The services are required to be provided from 01/04/2024 to 31/03/2026.
