Notice Description
Cheshire West and Chester Council and West Cheshire Integrated Care Board are looking to commission a Community Led Care and Carers Service for Older Adults and all age carers. The Community Led Care and Carers Service aims to encourage people to live as independently as possible with access to activities and support through the community, family, and friends. The Community Led Care and Carers Service will offer relevant information, advice, and support to help people develop their own solutions, choices and decisions to achieve what is important to them and meet their identified outcomes. The Council wish to develop an effective, co-ordinated, hub and spoke model of excellent quality advice provision which meets the needs of the borough's residents. The Hub and Spoke model will be a collaboration of organisations, to work together to strengthen the offer available to vulnerable young people, adults, Older Adults, and Carers in West Cheshire.
