Community Connect/Social Prescribing Services This contract will be awarded to an existing provider following the direct award process C. The approximate life time value of the contract is PS660,000 Contract start date: 01 October 2024 Contract end date 30 September 2026

Options: The initial term of the contract is 2 years plus an option to extend for an additional 2 years.
