Bracknell Forest Council is looking to commission an information, advice and guidance Stroke Support Service on behalf of the joint East Berkshire partnership. This includes Bracknell Forest Council, Slough Borough Council, the Royal Borough of Windsor and Maidenhead and Frimley Integrated Care Board. The Care Act 2014 places a duty of care on local authorities to provide information and advice to promote the wellbeing of individuals, including stroke survivors, ensuring that people receive the support they need to live independently and make informed choices about their care. The service provides practical advice, emotional support and high-quality information to stroke survivors, their family members and carers who are residents in Bracknell Forest, Slough or Windsor and Maidenhead. A variety of support options are provided to people which includes home visits, video calls and telephone calls. A person-centred approach allows the person to decide which approach is best suited for them. The service will be expected to liaise with key stakeholders to receive and action referrals. Key outcomes for the service will include, but not be limited to, the following: * Enhanced quality of life * Increased confidence, self-esteem and self-efficacy * Increased physical and mental wellbeing * Reduced risk of secondary stroke and other cardiovascular events * Increased social connection, participation and involvement in purposeful activity * Enhanced knowledge, skills and confidence to self-manage ('activation') * Increased control, choice and independence * More positive experience of care which is personalised to them, their needs, preferences and individual circumstances. We would like to draw on providers' expertise to inform the review of the specification and service model, with the attached questionnaire.
