Outsourced fully managed Dermatology service which will include all current activity. This contract will be for a period of four (4) years. The Dermatology service will be Consultant led with activity based at theTrust or alternative off-site provision within Milton Keynes The referrals and activity for this secondary level care Dermatology service fall under the below sub-specialties:
* General dermatology
* Cancer 2-week wait
* Adult Skin Tumour Service
* Paediatrics
* Gynaecological skin service
* Eczema, Psoriasis, Acne
* Patch testing
* Phototherapy
* Camouflage
* Dressings
* Iontophoresis

Dermatology procedures, undertaken as outpatient include:
* Excisional biopsy
* Diagnostic biopsy including- Shave, punch and Incisional
* Nail removal/Biopsy
* Curettage and Cautery
* Local flaps and Grafts

The service provider will work closely with local general practice, commissioners, and other care providers to develop and implement a sustainable model of care across both an adult and paediatric dermatology service provision delivered in line with the Dermatology Acute Service Specification 2023 (British Association of Dermatology)

Level 3 Specialist Care
This is a consultant led service delivered with the required multidisciplinary and professional specialist expertise. The clinical interventions and care pathways delivered at this level of service requires appropriate levels of supporting clinical and service governance.
Care within this level should be provided according to clear pathways and evidence based national protocols agreed with the relevant specialty consultants. This will include the treatment of complex patients, including:
* Working closely with other services to provide effective support and empowerment to complex patients and carers with multiple long-term conditions, or co-morbidities to manage long term or chronic conditions, including supporting the identification of these patients and ensuring rapid access processes are in place for these patients.
* Provision of effective public education, including that related to prevention, self-care and general wellbeing, through a number of messaging channels e.g. posters, leaflets, social media, schools and colleges, and other healthcare services.
* The service provider will support continuous improvement and GP education through the development and implementation of training programmes for practice staff including practice nurses, GPs, specialist GPs and pharmacists. This will include training and education in prescribing effective drugs, topical solutions, and focus on empowering patients to self-care.
* Dedicated electronic and telephone contacts for GPs to access specialist dermatological advice, in order to support patient management in a timely manner. This may be delivered via the E-Referral Systems Advice and Guidance or take the form of using innovative technologies supporting telephone-based services, video based teledermatology services or another model to deliver these outcomes, to support referral management and management of patients in primary care.
* Rapid access to specialist clinics including urgent, cancer and emergency care. This will include supporting inpatient dermatology referrals.
* Provide a Phototherapy/ PUVA service to Milton Keynes patients.
* Provide a patch testing service to Milton Keynes patients that meets the BAD (British Association of Dermatology) and the BSCA (British Society of Contact Allergy) accredited service standards.
* Biologics and Immunosuppressant therapies for the treatment of chronic inflammatory skin disease.
* Paediatric dermatology services.
* Multidisciplinary team input of care and management of chronic skin conditions, unresponsive to treatment.
* Care for complex skin problems within multidisciplinary clinics e.g. with Geneticists, Surgeons, Rheumatologists or Gynaecologists.
* Day care treatments for infusion therapies - modifying drugs and treatment, skin cancer surgery.
* Advice on the management of skin problems in patients admitted with other illnesses/disease.
* Psychological assessment of patients with skin conditions with referral to relevant specialist for care.
* Diagnostic investigations for rashes with systemic disturbance.
* Patient entry into Clinical trials where appropriate for skin disease teaching, research, training and assessment of medical students, doctors and other healthcare professionals are an important part of this service.
* The service provider is expected to ensure that the appropriate dermatological clinical representation is provided for any MDT arrangements necessary to deliver the standards of care required by the Commissioner including skin MDT meetings.
* The service provider will ensure that outpatient services are accessible and have the capacity to meet demand, which may fluctuate due to seasonality and other factors in delivery of cancer and overall RTT performance.
* The service will include follow up management when clinically indicated with an appropriate clinician (either telephone or face-to-face) in line with NHSE PIFU initiatives or similar. Follow up appointments will not be routinely offered.
* Provision of nurse led services for appropriate long-term conditions such as eczema and psoriasis, with a focus on patient empowerment.
* Clinical oversight for specialist nurses is expected to be provided by a lead nurse.
* Service developments would be encouraged to expand local services for patients, for example Mohs surgery. Additional information: This is a Provider Selection Regime (PSR) (The Most Suitable Provider Process) intended approach notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award.'
