Justification for urgent award There was a backlog of Fast Track reviews as part of the All Age Continuing Care Service, the backlog of reviews meant that patient needs had not been reviewed in a timely manner, resulting in patient care not being optimised, there was also a patient safety risk due to lack of timely care reviews.
