The service promotes people to use community opportunity services as a transition to independent living / independence and to operate with a preventative focus. The initial duration of the model is 5 years but may run beyond this term if it continues to be fit for purpose.

Community Enablement Services

This will be a service for people with a range of needs. This may include, learning disabilities, sensory impairment, autistic spectrum disorders, multiple physical disability, epilepsy, dementia and additional mental ill health. Individuals with acquired brain injury may where appropriate be referred to the service. These services will support people to increase their employability and independence skills whilst accessing community groups to expand their social circles and potentially their network of support. The aim is that the support will be provided for up to six months to support and increase the persons confidence to access their community and beyond. The intention would be to reduce the reliance on paid support, and this could also become part of the Adult Social Care Prevention Team offer when someone approaches the Council for support. These services are intended to support people with higher levels of support needs. These services will support people to increase their employability and independence skills whilst accessing community groups to expand their social circles and potentially their network of support. The aim is that the support will be provided for up to twelve months to support and increase the persons confidence to access their community and beyond. The intention would be to reduce the reliance on paid support over time. Please note, once this notice has been published, it can take up to 3 hours to appear on the YorTender portal. These services will replicate and expand on the current building based and outreach support for people with multiple and profound disabilities (Complex Needs). This Lot includes an ambition to develop the market and explore innovative offers to provide support for Carers to continue in their caring role, by providing evening and overnight opportunities for the people they care for. The overarching aim is to develop additional innovative offers to increase the choice and type of respite available in Rotherham to Carers and those they care for. The Council has now published the application documentation and notices to the market via YORtender (YORtender ID:53601) and encourages support providers to apply. Organisations can register free of charge and download the application documentation. A market engagement event for potential applicants is being held on: Tuesday 25th March at 1pm at Rotherham Town Hall (John Smith Room), S60 2HB. This is an opportunity to provide an overview of what the Council is looking for and how to apply. Services may be delivered online, individually or within groups, as appropriate and subject to individual choice. The FPS will remain open to new applicants and providers can bid for one or multiple Lots: Lot 1 Community Opportunities Enablement Services (up to 6 months) Community opportunities under this category will be time limited with clear goals and outcomes around identifying and meeting personal aspirations, supporting the person to gain or regain skills (e.g., independent living skills, or skills that will enable them to get paid work), connecting the person with local groups, services and interests. Packages under this category will be for an initial period of up to 12 weeks. Lot 2 Community Opportunities Personalised Services (up to 12 months) Service delivery for People with more complex needs, and/or frailties with the aim of offering goal-oriented support to help them become more independent, feel more confident and maximise their abilities. Services may be offered for a longer period dependent on the person's needs and aspirations. Lot 3 Community Intensive Support Opportunities (Complex Needs) These services will replicate and expand on the current building based and outreach support for people with multiple and profound disabilities (Complex Needs). This Lot will cover services for people with learning disabilities and autistic people, but may also include sensory impairment, physical disability, epilepsy, dementia and additional mental ill health.
