Improving health outcomes by encouraging clients to self-mange their conditions, reducing social isolation and dependence on unplanned use of health and social care services.

The ICB are intending to award a contract to an existing provider following direct award process C. The services are intended to be provided for a period of one year from 01st April 2025.
