Notice Description
Cheshire West and Chester Council are currently reviewing their reablement process for individuals transitioning from hospital to home-based domiciliary care. To assist with our soft market testing, we would appreciate your feedback via the questionnaire published on the Chest, www.the-chest.org.uk. We may wish to meet with you to discuss your responses or experience, please indicate whether or not you would be happy for us to contact you at the end of the questionnaire.
