Discharge to Assess Test for Change
Notice Description
Aberdeen City Health & Social Care Partnership wish to engage a social care provider to develop and deliver a Discharge to Assess Test of Change. This will be run over a maximum period of two years, with built in monitoring and evaluation to ensure that outcomes are being met. The Test of Change will be delivered by a dedicated team who would work alongside hospital staff in Aberdeen Royal Infirmary to identify individuals who are medically fit to be discharged but require a period of recovery, assessment and enablement in their own home environment. The level of need of these individuals would normally indicate that they would require a care home environment. On discharge the service will provide an intensive, targeted period of support and assessment at home, including wrap around 24/7 support where required. The service will have an enablement focus and include an assessment of a person's ability to live at home and the level of ongoing support required to enable this. It is anticipated that following this period the supported person would be able to remain in their own home and avoid alternative accommodation.

The Test of Change will be delivered by a dedicated team who would work alongside hospital staff at Aberdeen Royal Infirmary to identify individuals who are medically fit to be discharged but require a period of recovery, assessment and enablement in their own home environment. The level of need of these individuals would normally indicate that they would require a care home environment. On discharge the service will provide an intensive, targeted period of support and assessment at home, including wrap around 24/7 support where required. The service will have an enablement focus and include an assessment of a person's ability to live at home and the level of ongoing support required to enable this. It is anticipated that following this period the supported person would be able to remain in their own home and avoid alternative accommodation. Scope: It is anticipated that the test of change will deliver around 500 hours per week within 6 months of starting service delivery. If the project is evidencing successful outcomes there will be scope to increase the hours of delivery to meet the needs of the Partnership. Timeline: It is expected that work on developing the project and recruiting staff will commence immediately following the appointment of a provider. Service delivery should begin within at least the next three months. The maximum budget for Year 1, including set up costs and assuming service delivery at 500 hours per week would be 800,000GBP. This would be subject to change should the number of hours delivered increase. The key elements of the service will be: - The service will be registered with the Care Inspectorate. - Support will be delivered 24 hours a day, 7 days a week as required. - A physical presence in Aberdeen Royal Infirmary to support close working with hospital staff, patients and families. - Provision of a care at home service for a targeted short period of time, which may include 24/7 support. Activities undertaken will include assessment and support planning, personal care, medication administration, support with daily living activities and signposting/supported referrals to other health and social care, statutory or community services as required. - An enablement focus designed to develop the independence of the supported person. - Support delivered will be continuously reviewed against the supported person's identified goals and will dynamically change on a visit-by-visit basis as required. - Assistive technology will be used as required - Liaison with Social Work teams to support the arrangement of ongoing packages of care as assessed. - A staff team which will include: - A dedicated Registered Manager who will have responsibility for overall management and development of the service - Team Leaders/Senior staff - Support Workers who will deliver support in line with support plans. All staff will have the appropriate qualifications, training and SSSC registration. - The provision of monitoring reports as required, attendance at regular monitoring meetings, and involvement in evaluation of the test of change at key points.
