Notice Description
The Local Government Association delivers the BCF Support Programme, providing support to local areas with the integration health and care services. The Programme is seeking a contractor to provide targeted support to Hampshire and Isle of Wight to improve both outcomes for people and for resource use in systems by reducing emergency admissions, discharge delays and care home admissions.
