Notice Description
The Local Government Association delivers the BCF Support Programme, providing support to local areas with the integration of health and care services. The Programme is seeking a contractor to provide targeted support to East Sussex; Brighton and Hove; and West Sussex, to improve outcomes both for people and for resource use in systems by reducing emergency admissions, discharge delays and care home admissions.
