Notice Description
The Local Government Association delivers the BCF Support Programme, providing support to local areas with the integration health and care services. The Programme is seeking a contractor to provide targeted support to Hampshire and Isle of Wight to improve both outcomes for people and for resource use in systems by reducing emergency admissions, discharge delays and care home admissions. Renewal: The Head Contractor will be entitled at its absolute discretion to extend the Contract Period on the same terms up to 31st March 2026, subject to business approval and funds availability. The terms and conditions of the Sub-Contract shall apply throughout any such extended period. The duration of this Contract shall be no longer than 9 months in total.
