Reablement Service (Intermediate Care - Pathway 1) in Somerset - Discharge to Admission (D2A)
Notice Description
Pathway 1 in Somerset is the Discharge to Assess (D2A) service and is the route for all supported discharges back to usual place of residence, excluding fast tracked End of Life. The Discharge to Assess model ensures no assessment for ongoing care happens in an acute hospital setting but happens in a person's usual place of residence instead. This enables speedy discharge and prevents the overprovision of long-term care. The ethos of the service is to provide high quality personalised reablement, to be outcome focused, and to enhance what the Individual can do themselves or with help from their support network.

Lot 1 - Mendip
Provisions of a Reablement Service (Intermediate Care - Pathway 1) - Discharge to Admission (D2A), in the Mendip area of Somerset. This Notice is an Intention to Award a Contract under the Competitive Process of the Provider Selection Regime (PSR). Approx. lifetime value: PS15,540,000 This is a recommissioning of an existing Contract with a new provider taking on the service.
Renewal: There is an option to extend by two further periods of up to 24 months each and one further period of up to 12 months.

Any written representations regarding the award of these contracts should be sent to: stephen.barker@somerset.gov.uk Award Decision makers: Iona Brimson - Head of Service Older People and Intermediate Care - Commissioning James Sangster - Service Manager (Older People Services) Lynn Stephens - Head of Service, HIS, Intermediate Care and CHC Tamsin Carey - D2A Manager/ Clinical Specialist Occupational Therapist Kirsty Ash - Head of Urgent and Emergency Care Commissioning No Conflicts of Interest were identified. Relative importance of Key Criteria: Social Value 5% - although Somerset Council's standard is to allocate 10% to Social Value, it was felt in this case that more should be allocated to the other Key Criteria and so this was reduced Value Pass/ Fail - this was a pass/fail as Applicants were required to accept the proposed hourly rate in order to bid Improving access, reducing health inequalities and facilitating choice 40% - this was seen as the most important Key Criteria for the service in question and therefore attracted the highest weighting Quality and innovation 30% - having a good quality service with the ability to develop/ innovate was also seen as very important and given the second highest weighting Integration, collaboration and service sustainability 25% - also a key area and therefore only a slightly reduced weighting from Quality and innovation",
