The service will accept referrals from the Rough Sleeper Initiative Team via Medway Councils referral system (Humme) and meet with clients before they move into the accommodation to discuss expectations. It will assist with all aspects of setting up the client's tenancy and provide flexible support, which may include working outside of regular office hours to better meet the clients' needs. The service will advocate for clients, ensuring they are connected to local support services and accompanying them to appointments when necessary. To ensure the highest standards of service delivery, the following quality requirements must be met: Technology: the service must utilise up-to date technology for case management. This includes secure database for client's information, mobile devices for staff to ensure accessibility. Training: Staff must have relevant training and experience in working with complex clients that may not be seen as willing to engage in support. Out of Hours Provision: The service must include a reliable out- of- hours support system to address emergencies and provide continuous support.
